Family Forms

The Circuit Court’s Family Civil Division has jurisdiction over domestic relations matters such as adoption, dissolution of marriage, child support, domestic violence support and more. These form packets are made available here to assist citizens who choose to represent themselves in Family Law cases and/or cannot afford an attorney. These forms have been developed and approved by the Florida Supreme Court for use in Family Law cases.

The Florida State Courts System’s Self-Help Center is an additional online guide to help direct you through the court system. The role of the state’s self-help center staff is to direct interested individuals to the self-help website where they can explore resources needed to represent themselves, access the courts, and other essential resources. The website linked below includes separately listed family law forms approved by the Florida Supreme Court. These same forms are provided in packet form (compiled) on the Clay County Clerk’s Office website. Additional state resources found on this state website include a directory of local self-help centers, web links to free and low-cost legal aid, mediator search capabilities, and guardianship resources at the state level.

FLHSMV Scam Text Notifications- We have been notified of a recent text scam circulating that claims the recipient has an outstanding traffic ticket that must be paid or they will be reported to the DMV violation database and their vehicle registration and driving privileges will be suspended unless payment is made.  The scam has spread across multiple counties throughout the state including Clay County.  Do not click on any links or make a payment should you receive a text from the Department of Motor Vehicles.  The FLHSMV does not send text notifications for payments.
